Leaves are turning, sweaters are out, and for a limited time, I'm serving up pumpkin spice medical reports: the same clear, thorough analysis you count on, with a little extra fall flavor.

Good news: I found the fix for the AI glitch! Turns out, two sets of eyes really do beat one, especially when one of them has actually stood at the bedside. Let's work smarter, not harder!

If your office is using AI to draft first-pass merit reports on new intake files, I want to flag something before one of those reports drives an intake decision that costs you money. AI-generated merit assessments are increasingly common, and increasingly wrong in ways that are hard to catch unless you already know the medicine.

Three things I see routinely when I put a clinical eye on an AI-drafted merit report:
Hallucinated findings: The model will state a lab value, a vital sign, or a documented complaint that isn't in the record because it generates plausible-sounding medical language rather than verifying it against the chart.
Invalid or misapplied standard of care language: AI defaults to generic, textbook standards. It cannot weigh the treating specialty, the hospital's own protocols, jurisdictional nuance, or a recent guideline change. A merit report stating a provider "breached the standard of care" without a nurse or physician confirming that standard actually applies is a liability in itself, not a strength, if it ends up in your file.
Missed clinical context: AI reads records linearly. It will not flag the vitals trend that should have triggered escalation, the contradiction between two providers' notes, or the late entry that changes the whole timeline; those are usually exactly what makes or breaks merit.

That is why I am offering a Second Look review. Before a merit report generated by AI goes into your file or gets relied on for an intake decision, I check it line by line against the underlying medical records and flag anything fabricated, unsupported, or clinically off, along with anything the AI missed that strengthens or weakens the case.

Flat fee: $500, covering records up to 1,500 pages or up to 5 hours of review time, whichever is reached first. If the records or review time exceed that, I will let you know before continuing, and I'll bill the remaining work at $150 per hour.

Attorney panicked after AI botched the merit report!

Resolved in under 11 hours!

Last week, an attorney called me in an absolute panic. He'd handed his case off to AI to build the medical chronology and merit review, thinking it would save him time and money. You know, bypass the LNC and go straight to the experts.
Instead, it handed him back an organized mess. He still couldn't tell you the complete liability theory because it only caught the major obvious mistakes. He couldn't tell you where the red flags were, because half of them weren't even in there. The case just didn't make sense. The dots weren't connecting, and the clock was ticking.

Sound familiar?

Here's what I did. I took 13,000 pages of records from three different facilities and built him a clean, chronological timeline: before, during, and after the events that mattered. I nailed down every viable liability theory, mapped out each defense he needed to be ready for, and flagged all red flags in those records, including several that were lost in the sauce the first time around. I found the two specific experts he needed and determined what records were missing, including the audit trail. I can't wait to dig into that!!

Total time: under 11 hours

AI can process pages. It cannot practice medicine. It doesn't know what a missed telemetry alarm means at 3 a.m., why a delayed response to a critical lab value matters, or which "unremarkable" chart note is actually hiding the whole case. That takes 25+ years of being in the room when it happens: ICU, ER, surgery, orthopedics, endoscopy, and pre- and post-op. That's the difference between a stack of summarized pages and a case you can actually try.

Legal nurse consultants belong in criminal cases too!

Here's where 22+ years of ER, ICU, GI, Ortho,surgery, and pain procedure experience earns its keep on that side of the courtroom:

Assault and homicide cases: I read the autopsy and medical examiner findings and check whether the mechanism of injury actually matches the medical evidence. Example: a stabbing case where the defense needed to know if a single wound could realistically account for the blood loss and time to death described by the state's expert. Example: a homicide collision where the driver's erratic behavior was blamed on intoxication, when the medical record pointed to a seizure or a sudden drop in blood sugar as the more likely cause.

Child and elder abuse cases: Bruising patterns, fracture timelines, and healing stages tell you accidental from inflicted, if you know how to read them. Example: a spiral fracture in a toddler with no matching fall history is a very different story than a straight fracture from a witnessed tumble; that distinction can make or break a case.

DUI and toxicology cases: Blood draw procedure, chain of custody, and whether medications or health conditions could skew a toxicology result. Example: a diabetic client whose low blood sugar mimicked intoxication on field sobriety tests, or a blood draw that sat too long before it was refrigerated and processed.

Competency evaluations: Organizing medical and psychiatric history so attorneys have the full clinical picture. Example: pulling years of scattered records into one clean timeline so the attorney and the evaluator both see the same picture, instead of piecing it together mid-hearing.

Medical neglect charges: The same standard-of-care analysis I run on malpractice cases, applied to caregivers and facilities. Example: a nursing home neglect charge where the real question is whether staffing and documentation met the standard, not just whether a bad outcome happened.

In custody deaths: Reviewing the medical care a person received against the standard of care for that setting. Example: did jail medical staff respond appropriately and within a reasonable time to a documented complaint of chest pain, or is that where the chain of care actually broke down?

Is “continue to monitor” hiding the real story in your case?
As an LNC, I often see “continue to monitor" as a red flag when reviewing medical records.
Most admission orders already include things like vital sign call parameters, so "nobody said what to watch for" rarely holds up once you're in the chart. The real exposure is documentation that doesn't keep pace with the patient. A nursing note that says, "continue to monitor patient" tells a jury nothing. Compare that to a nursing note like this: "Temp 100.8 at 1400, up from 99.2 at 1000; MD notified per parameters, no new orders at this time, will recheck at 1800." Or a provider note that says, "Monitor temperature q4h; call provider if greater than 101.5F or if patient develops tachycardia or altered mental status." Both of those show the nurse and provider saw the trend, acted within the standing parameters, and documented the clinical reasoning behind the plan.
That gap, between a documented trend with a response and a phrase repeated while the flowsheet tells a different story, is exactly what I'm looking for buried in countless locations within the chart. It's rarely one dramatic error. It's whether the record shows anyone was actively reading and monitoring the data as it changed. If your case has a "continue to monitor" note buried in it, that's worth a closer read before you decide what the chart supports.
