When determining the capacity of an adult when determining if guardianship is required, a Capacity Assessment Report is required to be completed.
A Capacity Assessment Report is is a formal medical and legal document evaluating an adult’s mental and cognitive ability to make independent decisions regarding their finances, health, or personal life.
The evaluation is conducted by an authorized, designated healthcare professional and is a mandatory requirement for applying for or reviewing adult guardianship, trusteeship, and personal directives.
