A diagnosis, a new prescription, or a discharge plan can be hard to understand, especially in a stressful moment.

This Health Literacy Month, we recognize the nurses, pharmacists, physicians, interpreters, and community health workers who take the time to explain care in plain language.

Their patience helps patients and families leave with confidence.

After a nursing home fall, families are often told it was an unavoidable accident.

The facility's own records usually tell a more complete story.

Before a fall ever happens, staff are required to assess each resident's fall risk and build a care plan around it, from non-skid footwear to assisted bathroom trips to a bed set at a safe height.

In every fall case, our team asks three questions:

1️⃣ Did the facility know your loved one was at risk?
2️⃣ What did the care plan require?
3️⃣ Was that plan followed?

As autumn rain moves in, those questions extend to wet entryways and slick floors near facility doors.

If a fall seriously injured your loved one, getting answers is possible.

Every person living in long-term care deserves to be treated as the individual they are.

Residents' Rights Month recognizes the residents, families, and caregivers who keep dignity, choice, and independence at the heart of care.

We are grateful for each of them, this month and always.

Flu, RSV, and COVID-19 cases climb every fall, and nursing homes know it is coming.

Federal regulations require every certified facility to run an infection prevention and control program led by a trained infection preventionist.

When residents show symptoms, staff must act quickly:

▪️Isolate sick residents or group them on a dedicated unit
▪️Keep contagious employees away from resident care
▪️Track symptoms daily and test when illness appears

When isolation is delayed or a unit is too short-staffed to separate sick and healthy residents, one case can quickly become dozens.

If an outbreak harmed your loved one, you deserve to know whether it could have been contained.

The people who raised us, taught us, and built our communities deserve our gratitude every day.

On the International Day of Older Persons, we honor their wisdom, their stories, and the countless ways they continue to shape the lives around them.

Take a moment today to call, visit, or simply listen to an older person you love.

Proving nursing home neglect requires turning scattered records into concrete legal evidence.

When care plans are ignored, proof is hidden in missing chart entries, unlogged timestamps, and incomplete shift handoffs.

Reconstructing care timelines exposes where protocol failures directly caused injury.

Seeking clarity after a care failure protects your loved one and future residents.

Medical procedures carry inherent risks, but there is a clear legal line between a known complication and medical negligence.

When a healthcare provider strays from accepted safety checklists or ignores post-operative vitals drops, that deviation can constitute malpractice.

Investigating surgical logs and expert standards reveals whether care failed.

If you suspect medical negligence altered your life, answers exist.

Nursing home falls are rarely just accidents.

During Falls Prevention Awareness Week, know that facilities must execute a 3-Step Fall Prevention Assessment for high-risk residents.

Key fall prevention requirements:

• Routine mobility and balance evaluations
• Verified bed height, mat placement, and call button positioning
• Strict adherence to required 1 or 2-person assistance plans

If a fall caused severe injury to your loved one, holding negligent facilities accountable is possible.

When an elderly resident experiences rapid physical decline, facilities often dismiss it as aging. In reality, it is frequently caused by discontinued physical therapy.

Facilities are required to provide prescribed mobility support to prevent avoidable regression.

When staff neglect range-of-motion exercises or walking schedules, it can result in severe joint stiffness and immobility.

If neglected therapy caused unmonitored physical loss for your loved one, you have options.

Timely medication administration is a mandatory safety requirement in long-term care.

Skipping prescribed doses or dispensing wrong amounts violates medical standards.

Facilities must maintain detailed Medication Administration Records (MARs) logging dosage, time, and staff credentials.

Gaps in these logs often reveal systemic understaffing.

If medication errors harmed your family member, guidance is available.
