A South Florida family is grieving after a tragic playground accident claimed the life of their 13-year-old boy. Olvin Cabrera Valladares was found trapped inside a tube slide at Shenandoah Park in Miami on Sept. 25. Family members say his shirt became caught on a screw inside the slide, causing the collar to tighten around his neck. Another family claims they reported safety issues with the same slide at Shenandoah Park months before Olvin's death. Playground accidents occur due to defects, lack of supervision and warning labels, failure to maintain the equipment or lack of safety ground cover. “I’ve done this job my entire adult life, and I think this is the most horrific bus tragedy we’ve had in decades,” Polk County Sheriff Grady Judd said. Two people were killed, including a 10th-grade student, after a school bus crash in Winter Haven yesterday. The bus was traveling eastbound on Lucerne Park Road when a westbound white Chevy Malibu crashed into it. The impact caused the bus to lose control and tear off a tire. The bus then collided head-on with a Jeep and ultimately overturned. Dozens of people were also injured in this horrific collision.

A school bus filled with children rolled over in a multi-vehicle crash Monday morning in central Florida, killing two and injuring multiple, according to authorities.
