Advanced Directives provide you with the opportunity to let your healthcare providers know--in advance--what treatments you want to receive and those you do not. Well, the snow and cold should be over soon and warmer temperatures will provide the opportunity to open your residential pool. Is your pool safe? Pools are an attractive nuisance to young children and especially children on the autism spectrum disorder. Autistic children are 160 times more likely to drown than their normal counterparts. For reasons that are not fully understood, autistic children are drawn to water. Is your pool properly secured? Is it really safe to prevent an accidental drowning? Most likely, it isn't. Please do what you can to protect your children and the children who live around you!

Medication Errors:

In a recent study in three tertiary care centers, 10,070 medication orders were written, and 530 medications errors were identified (5.3 errors/100 orders), for a mean of 0.3 medication errors per patient-day, or 1.4 per admission.

Adverse Drug Events:

Of the medication errors found by pharmacists and nursing staff, 53% involved at least one missing dose of a medication; 15% involved other dose errors, 8% frequency errors, and 5% route errors. During the same period, 25 ADEs and 35 potential ADEs were found. The study found that all of the ADE's were preventable!! Physician computer order entry could have prevented 84% of non-missing dose medication errors, 86% of potential ADEs, and 60% of preventable ADEs.

When you are in the hospital, be certain you are getting the medications you are supposed to be taking and when you are to be taking them. Don't be afraid to advocate for yourself and question medications. Perhaps the life you save may be your own!!!!!
