Two residents at Bria of Wood River with tracheostomies died after staff performed CPR incorrectly by placing breathing bags over their mouths instead of their tracheostomy tubes because staff had never been trained how to resuscitate someone with a tracheostomy and either didn't have the correct equipment or didn't know how to use it.
