Regarding COVID-19 and care management

Dear Clients and Families,
During these trying and uncertain times, the health and well being of all of you is a top priority for us. In response to COVID-19, we will be changing some of our practices to reflect local, state and national health directives.
Hospitals, skilled nursing homes and senior communities have restricted visits and these guidelines are sure to evolve as more information is available. We will respect these restrictions. For the time being we will discontinue our visits in the community except for essential visits, agreed upon with our clients and their families. We will be employing the prevention practices recommended by the U.S. Centers for Disease Control including frequent hand washing with soap and water, using hand sanitizer when hand washing is not feasible and social distancing. Please follow these guidelines for your health as well. Thoughtful preparedness and calm responsiveness are always our best defense.
Please be assured that despite any limitations that may be imposed in care settings and facilities, our team is working to implement resourceful adaptations so we can continue to meet the needs of our clients. This includes telephone contact and conferencing, and availability to run errands if needed.
If there is any possibility that you may have been exposed to COVID-19 (Coronavirus), we ask that you refrain from meeting with our staff face-to-face and advise you to seek alternative plans to connect via telephone. Your cooperation with this request will ensure that we will be able to continue to serve our clients while reducing the risk of further spread.
As always, we value and appreciate our clients, families and providers. It is our goal to continue supporting each other during this challenging time. We are confident we will weather this storm together.
