Job Summary
The Case Manager plays a crucial role in providing support and guidance to individuals facing various challenges, including mental health issues, addiction, and social barriers. This position involves assessing client needs, developing personalized care plans, and coordinating services to ensure clients receive the necessary resources for their well-being. The ideal candidate will possess strong interpersonal skills and a deep understanding of behavioral health and social services.
Responsibilities
-Conduct comprehensive assessments to identify client needs and challenges.
-Develop, implement, and monitor individualized care plans in collaboration with clients.
-Provide crisis intervention and management to clients in distress.
-Offer addiction counseling and support to individuals struggling with substance use disorders.
-Facilitate access to behavioral health services and community resources.
-Mentor clients in developing life skills and - achieving personal goals.
-Supervise case management staff and provide guidance as needed.
-Collaborate with external agencies.
-Maintain accurate documentation of client interactions, progress, and outcomes.
-Participate in workforce development initiatives to enhance client employability.
Skills
-Proficient in crisis intervention techniques and crisis management strategies.
-Knowledgeable in addiction counseling practices and behavioral health principles.
-Strong background in social work with experience in child welfare systems.
-Familiarity with motivational interviewing techniques to encourage client engagement.
-Experience supervising staff or mentoring individuals within a social service context.
-Understanding of childcare principles and resources available for families in need.
-Ability to navigate Medicare systems and advocate for client access to healthcare services. -This position requires a compassionate individual dedicated to empowering clients through effective case management practices while fostering a supportive environment for personal growth and recovery.
Job Type: Full-time
Pay: From $47,000.00 - $56,170.00 /year
