LTD Driving School Student:
Just a reminder, there are several items that you will need to take to Las Cruces the New
Mexico Motor Vehicle Division in order to receive your provisional license. You may
also go to the White Sands and Sunland Park MVD to obtain your provisional license for
an additional service fee. The following is a list of what you will need:
1. Green Certificate of Completion (make a copy for your insurance company)
2. N.M. Learner’s Permit
3. Original Social Security Card or Matricula Consular (not laminated/ taped together)
4. Completed Driving Log (50 hours of behind the wheel training)
5. Parent or Guardian (if under 18)
6. $20.00 (for the license)
7. Driving School Record of Completion Form
Please note, you may go to get you provisional license 6 months after the date of issue of
your N.M. driving permit. And you need to get your provisional license ASAP, the test
scores are only good for six months. Otherwise you will have to be retested at the MVD.
There is a $30.00 replacement fee for lost or destroyed certificate of completion. If you
have any questions please do not hesitate to call. Once again thank you for your
