🌟 Guardian Case Manager – Join Our Mission Driven Team
Amaral Associates, Inc. is seeking a compassionate, organized, and dedicated Guardian Case Manager to join our small, mission focused practice. We support adults who require assistance with decision making due to cognitive, medical, or functional limitations, including individuals under guardianship and conservatorship. Our work centers on dignity, safety, and person centered advocacy — and we’re looking for someone who shares that commitment.
As a Guardian Case Manager, you will play a meaningful role in improving the lives of individuals who rely on us for support, guidance, and protection. This position is ideal for someone who is empathetic, detail oriented, and motivated by purpose driven work.
💙 What You’ll Do
• Build supportive, professional relationships with clients, providers, and care teams
• Communicate regularly with group homes, nursing homes, assisted living facilities, day programs, and medical providers
• Coordinate medical, behavioral health, and community based services
• Monitor client well being and ensure their rights, preferences, and safety remain central
• Attend team meetings, care conferences, and — when appropriate — medical appointments (in person attendance is helpful but not required)
• Document all contacts, decisions, and updates in a timely and accurate manner
• Assist with benefits coordination, service authorizations, and care planning
• Advocate for clients’ needs with professionalism, clarity, and compassion
💼 What We’re Looking For
• A strong sense of empathy and a genuine desire to support vulnerable adults
• Excellent communication skills — written, verbal, and interpersonal
• Ability to navigate complex situations with patience, professionalism, and sound judgment
• Strong organizational skills and comfort managing multiple priorities
• Experience in human services, social work, case management, or related fields is helpful but not necessary. We are willing to train the right person.
• Familiarity with guardianship and conservatorship is also a plus
