Before we notarized the document/s, you must comply with the following:

1. Personal appearance required.
Dapat personal na humarap sa notary public.
Bawal ang:
❌absent signatory
❌video call only
❌authorization lang
❌tagapirma lang sa iba

2. Competent evidence of identity.
🪪 Magdala ng AT LEAST ONE (1) CURRENT ORIGINAL and VALID government-issued ID.
No expired IDs. ❌🪪
List of acceptable IDs:
✅ Philippine Passport
✅ Driver’s License
✅ Professional Regulation Commission (PRC) ID
✅ Unified Multi-Purpose ID (UMID)
✅ National ID (PhilSys/ePhilID)
✅ Senior Citizen ID
✅ Overseas Workers Welfare Administration (OWWA) ID
✅ Voter's ID
✅ Government Service Insurance System (GSIS) e-Card / Social Security System (SSS) Card
✅ Postal ID
✅ National Bureau of Investigation (NBI) Clearance
✅ Police Clearance
✅ Person's With Disability (PWD) ID
✅ Alien certificate of registration card

Note: Community Tax Certificates (Cedulas) are not considered competent evidence of identity.

3. Document must be complete. 📋
No blank spaces.
Complete pages.
Complete attachments/ annexes.

4. Signature requirement.
For ACKNOWLEDGMENT:
Document may already be signed, BUT signatory must personally appear and acknowledge the signature.
For JURAT/ AFFIDAVITS:
Affiant must sign IN THE PRESENCE of the notary public.

5. If no VALID ID available:
1. The oath or affirmation of a third person who is personally known to the notary public and who personally knows the person; OR
2. The oath or affirmation of two witnesses who each personally knows the person and shows to the notary public their IDs.

‼️IMPORTANT REMINDER‼️
False statement under oath may result in criminal liability.
Hindi lang basta tatak ang notaryo. Once na mai-notarized ang document, it becomes a public document with binding legal consequences.
