Eligibility:
- Applicant(patient) must have recommendations or references for specialized treatment on the basis of Preliminary medical checkup and treatment from the Medical experts of Bangladesh. Official documents of recommendation from the Doctors there has to be obtained.
- The treatment should be significant in nature not all ailments qualify for a Medical Visa.
- One attendant should accompany the applicant(patient) (visa required). Not more than two attendants shall be issued a Visa and they both need to be blood relatives of the applicant(patient).
Documents required:
- Passport with 6 months validity
- NID/Birth certificate
- Passport size photographs - One recent (not less than 3 months old) 2x2 (350x350 pixels) color photograph depicting full face with white background.
- Photo copy of passport
- Proof of Residential Address (Latest electricity bill, by any name)
- Referral from Local Doctor
- All medical documents (doctor's advice, prescription, reports) of the treatments that are done (two sets - original and photocopy)
- Bank statement/Dollar endorsement (Endorsement should not be older than 1 (one) month at the time of submission) /Active international credit card /Travel card
- Medical Invitation letter from India with specific date
- Profession proof (Visiting card. Trade Licence/Memorandum. Service holders-certificate from the employer. In case of students, copy of Identity card from the educational institution is to be attached, for retired person need retirement papers)
- Passport copy of attendant (for Attendant visa)
- Proof of relationship with attendant (for Attendant visa)
- Last passport copy and all old passports
