Momentum matters, but growth comes from what you do every single day.

This year I am choosing discipline and consistency on purpose. Last year, some big projects pulled me away from my daily habits. Stress tried to push me back into old comfort zones. And while progress was still happening, alignment was not where I wanted it to be.

This year feels different. This year is about showing up even when it’s inconvenient. Doing the small things consistently. Building habits that support the vision, not sabotage it.

Sometimes compliance isn’t about working harder — it’s about thinking differently.

Recently, I had the opportunity to support a Pediatric Home Health agency during an OIG audit. During the review, it became clear that the agency did not have the appropriate codes documented on their plans of care to support billing for higher-level services.

Instead of stopping there, we took an outside-the-box approach. By identifying and leveraging alternative supporting documentation that clearly reflected the required codes, we were able to demonstrate medical necessity and compliance — ultimately helping the agency avoid a potential $30,000 loss.

But we didn’t stop with the audit.

Every result begins with a thought we rarely stop to question.

The way you interpret a challenge often decides the outcome before anything else happens. Not because the work is easy or hard, but because your mindset determines how long you stay engaged, how deeply you think, and whether you look for solutions or settle for assumptions.

Some thoughts deserve to be trusted. Others sound convincing but are simply untested beliefs.

I am not ready yet
I am not built for this level of responsibility
This is just the way it has always been

Those are not facts. They are narratives. And narratives can be changed.

When you choose to believe growth is possible, you approach problems differently. You stay curious longer. You ask better questions. You recognize that clarity often comes after effort, not before it.

There is truth in the idea that what you believe about yourself quietly shapes your results. Progress starts when you decide which thoughts are grounded in reality and which ones no longer deserve your agreement.

Recently, I assisted an attorney who was exploring a potential delay in care involving an oncology patient. They asked for help locating an expert to review the medical records and provide an independent opinion.

The first expert carefully reviewed the records and did not find sufficient evidence to support a delay in care. At the family’s request, a second expert was brought in for another opinion, and they reached the same conclusion.

I know firsthand how incredibly difficult it is to lose a loved one, especially to cancer. While the outcome may not have been what the family hoped for, I am grateful I could help provide clarity and a sense of closure during a very emotional time.

💡The Hidden Story in Medical Records

Most people see a stack of medical records.

I see a story waiting to be told — one that can make or break a case.

As a Legal Nurse Consultant, my role isn’t just to review records — it’s to uncover the why behind what happened.

🩺 Why did the patient fall?
📋 Were safety protocols truly effective?
👣 Could this have been prevented?

In one case, a patient was educated multiple times about using the call light for assistance. Despite that, she attempted to walk to the restroom alone — and staff entered the room just after she fell.

After the incident, she complained of shoulder pain. The MD was notified and an x-ray ordered.

On the surface, it may seem like a case of patient noncompliance. But when you look deeper, the real story is about the effectiveness of education and the hospital’s responsibility to ensure safety.

If re-education isn’t working, maybe it’s time for a different intervention — such as assigning a sitter or implementing closer observation.

As LNCs, we don’t just identify what happened.

We analyze how it could have been prevented.

We had an incredible success this week with one of our home care clients — and it’s a great reminder of how powerful proactive audits and strong processes can be. 🙌

Our client was under OIG investigation for concerns that UA modifiers were billed without proper diagnosis codes.

At first, the potential payback was estimated at $400,000. 😳

After our initial review, we provided MD orders that reduced that amount to $12,000. But we didn’t stop there.

🕵️‍♀️ We identified a process breakdown in how the company handled post-hospitalization orders — specifically when patients were discharged with ventilators. We discovered that orders weren’t being consistently processed during resumption of care.

By locating the ventilator orders directly from the DME company, we were able to close the gap in documentation completely.

✅ Final Result: The client went from a potential $400K payback ➡️ $12K ➡️ $0 — and a closed case.

Even better? They’ve now strengthened their resumption of care process to ensure this doesn’t happen again. 💪
